He claims that he's seen Shepard live under those circumstances. I've never seen it, but I can't really argue against it.

 

I did my Insanity run and had every crew member other than Joker and myself die, so I know it's possible. For some reason, I didn't do it with my imported evil character from ME1. That's my fault.
